Patients wishing to join the practice should come into the surgery and complete forms,GMS1 and patient profiling form which asks questions regarding your ethnic group, military service, and alcohol usage. Patients must have proof of address and photographic ID.
Summary Care Record (SCR) has been introduced to improve safety and quality of patient care. Because the Summary Care Record is an electronic record it will give healthcare staff faster, easier access to essential information about you, to help provide you with safe treatment when you need care in an emergency or when your GP practice is closed.
You can choose to have a Summary Care Record or Decline to have a SCR
1.Express Consent for SCR you do not have to do anything this will happen automatically
2.Express Dissent for SCR you need to let your GP practice know by filling in the Patient Profiling form.
You can change your mind at any time you need to let your GP practice know.
If you need anymore information on Summary Care Record the practice can provide you with a leaflet at reception.
You are now registered with this surgery.
New patients must be seen by the nurse within two weeks of completing this form.
